Learning objectives
This study focuses on technical aspects of MO.MA device,
which is a proximal protection system that can be used in Carotid Artery Stenting (CAS),
when exists severe stentotic with brindle atheromatous plaques,
or extreme arterial tortuosity.
Clinical and radiological aspects are also reviewed in this study in order to get a proper patient selection and a suitable safety profile.
Background
CAS is an accepted modality of intervention for treatment of carotid atheromatous disease.
Nevertheless its periprocedural morbimortality is a frequent focus of discussion and there is a permanent comparison to endarterectomy.
Lots of publications have shown the safety and efficacy of CAS even those in patients with a high surgical risk.
Furthermore,
the widespread use of protection devices has increased the safety of this intervention,
thanks to the reduction of intracranial embolisation of debris produced during manipulation of the atheromatous plaque.

The MO.MA device a ballon catheter,
used as guide catheter,
which is located in the CCA at the beginning of intervention.
It precises a 9F introducer sheath in femoral artery to assure the acces.
The device is a single catheter system (usable length of 95 cm) integrating the functional aspects of cerebral protection and a guiding sheath to perform stent implantation and removal of debris by blood aspiration.

Conclusion
The MO.MA device is a feasible system of protection in ASC which allows the treatment of atheromatous carotid lesions when conventional filter can not acces because of tortuosity,
or it is excessively dangerous cross it due to brindle plaques.
To perform adequately the ASC intervention with this protection device,
we consider it very important to make a proper pre-selection of patients,
paying special attention to a functional circle of willis.
