Purpose
to evaluate the interobserver agreement in the diagnosis of the lesions of the labrum with fat saturated Proton Density Fat Spin Echo (fs Pd FSE) and 3D FIESTA (Fast Imaging Employing Steady State Acquisition) at arthro-MRI.
Methods and Materials
In the period February -July2012 we performed 3D FIESTA (0.6 mm thickness,
0.3 mm spacing,
5'25'') in40 consecutive patients during arthro-MRI of the hip (at least fs PD FSE in three orthogonal planes and in radial acquisition) (Fig.
1-2).
The MR sequences were acquired after intrarticular injection of 12- 20 ml of contrast-media(Gd-DOTA solution for intrarticular use - gadolinium-tetraazacyclododecanetetraacetic acid).
The Fast Imaging Employing Steady State Acquisition (FIESTA) sequence provides images of fluid filled structures with very short acquisition times.

Results
Thefs PD FSE sequences allowed moderate agreement between the two observers (24 patients out of 40 patients: k-Coen: 0.49).
The 3D FIESTA reconstructions allowed good agreement between the two observers (36 patients out of 40 patients: k-Coen: 0.84).
Apartial chondrolabral detachment without degenerative changes (IV) was easily seen in fs PD FSE (Fig.
9-10) and 3D FIESTA reconstructions (Fig.
11-12) by both radiologists in this case.
In other casesthe presenceof labral disruption (II) is easily diagnosed (Fig.

Conclusion
3D FIESTA could make easier the diagnosis of the lesions of the labrum at arthro-MRI of the hip and it increases the interobserver agreement in our experience.
This sequence may be usefully added to routine protocol of arthro-MRI,
even if these results are still to be compared to the arthroscopic data.
