Purpose
The purpose of this study was to compare a high-definition CT (HDCT) with a conventional 64-row standard-definition CT (SDCT) angiography as to radiation dose,
image quality,
accuracy of vessel diameter measurement for imaging small caliber coronary arteries in vivo.
Methods and Materials
28 patients from different University hospitals with history of coronary artery disease has singed the informed concent form and scanned by HDCT (Discovery CT750 HD) and SDCT (Somatom Definition AS).
Totally 280 measurements from 140 coronary arteries were analyzed by 2 independent radiologist.The scan modes were bothaxial prospective ECG-triggered.
The vessel diameters of ascending aorta(AA),
left main coronary artery (LM),
left anterior decending coronary artery (LAD),
left circumflex artery(Cx),

Results
The intraclass correlation coefficient (ICC) of measured vessel attenuation values in SDCT between the two radiologists was very good.
The ICC was higher in HDCT according to inner and outer diameters of Cx,
LAD and also outer diameters of LM.
But ICC was higher in SDCT according to inner and outer diameters of AA and RCA.
First radiologist image quality score and the attenuation value measurements are higher than second radiologist in HDCT and visa versa for SDCT.

Conclusion
On the basis of this in vivo study,
HDCT has a higher radiation dose than SDCT but has much more atenuation and the spatial resolution which improve measurement accuracy for imaging coronary arteries.
