Purpose

To evaluate the gain in bone height and quality in patients with moderate and severe bone atrophy treated with dental implant by vertical osteotomy in maxillary bone.

Methods and Materials

In our study 30 patients underwent a pre-operative Cone-Beam-CT scan performed with Soredex Scanora 3D®. Patients were distributed in two groups, the first (A) including the ones with bone heightfrom 5 to 7 mm, vestibulo-lingual thickness from 7 to 9 mm and bone density D2 or D3 in Misch scale. The second group (B) included patients with bone heightfrom 2 to 4 mm, vestibulo-lingual thickness from 4 to 6 mm and bone density D3 or D4. All patients were submitted to a new CBCT scan...

Results

Comparing pre- and post-operative studies, group A (moderate alveolar atrophy) showed an average increase in bone height of 10 mm (68%), in vestibulo-lingual thickness of 15 mm (83%) and no significant decrease in bone density. Group B (severe alveolar atrophy) showed an average increase in bone height of 5 mm (33%), in vestibulo-lingual thickness of 7 mm (41%), but a significant decrease in bone density with an average of D2.

Conclusion

Dental implant by vertical osteotomy in patients with moderate bone atrophy determines a significant increase in height and bone thickness with no changes in bone density. In patients with severe atrophy, the gain in height and bone thickness is minimal, but it is relevant the improvement in bone quality, essential for the implant integration.
