Aims and objectives
Despite on the initiation embryonic malignancies of fetus in utero and the widespread availability of ultrasound (US) for pregnant in most cases embryonic malignancies are diagnosing in infants postnatally when there are a specific clinical symptoms and significant tumor volume.
Median age of the actual primary imaging embryonic malignancies such as neuroblastoma,
nephroblastoma and hepatoblastoma in children under one year according to own data is 6 months and the median tumor volume - 176 (39,5–355) cm³.
Currently,
by results of mandatory prenatal ultrasound is possible...
Methods and materials
Following an initial ultrasound diagnosis of malignant neoplasms of 109 children of both sexes aged 1–366 days were studied by volumetric and kineticcharacteristicstumors.
All patients were with confirmeddiagnoses of the nephroblastoma (n=31),
retroperitoneal neuroblastoma & neuroblastoma adrenal (n=58) and hepatoblastoma (n=20) morphologically (see Fig.
1-4).
Tumor volume (V,
cm³) was determined by ultrasound,
counting it's by formula V = 0.523aхbхc,
where a – length,
b – width,
c – tumor thickness (in cm) or in program VOCALTM (Virtual Organ Computer Aided anaLysis) - Fig.
5....
Results
There were a correlation between tumor volume and infants ages (r = 0.52; p<0.05). Nonlinear model using the Gompertz function describing the growth of the tumor was fitted using logarithmic form of equation (1) (Fig.
9)
: for time point t0 = 0 (day of birth),
and time point (prenatal diagnosis) t0 = –48 days (see Table1 in Fig.
10,
Fig.
11).
We obtained significant estimation of the tumor volume at time point of birth,
but non significant estimation at the 32-33th weeks of pregnancy....
Conclusion
Thus,
according to the mathematical modeling was established that malignancies growth rate in children under one year can to critically vary in the first months of life.
According to the results of statistical calculations and mathematical modeling were synchronized baby age and the time of realizing the malignancies metastatic potential.
In case of screening using ultrasound period of age 3-4 month suggests the possibility of earlier visualization malignant neoplasm in the first months of life in much smaller volumes than those that actually there are...
