Aims and objectives
Pulmonary embolism (PE) and deep venous thrombosis (DVT) together,
known as venous thromboembolism (VTE),
is one of the most common and most serious complications of patients with malignant tumors.
According to previous studies,
oncologic malignancy is an independent risk factor of VTE [1].
VTE is the second cause of death in patients with malignant tumors and indicates poor short term and long term prognosis [2,
3].
In recent years,
the incidence of PE detected in routine CT scan of patients with malignant tumors has been...
Methods and materials
1.
Subjects and groups
We retrospectively enrolled 1974 cases of hospitalized patients with digestive tract tumors (esophagus,
stomach,
small bowl,
colon,
rectum,
pancreas,
appendix) who had undergone routine contrast enhanced chest and abdominal CT from Jan,
2011 to Sep,
2014 in our hospital.
58 cases among them were found to have UPE according to contrast enhanced CT images.
108 cases without UPE who were 1:2 matched with UPE group respectively in age,
tumor site,
pathological type and tumor stage were enrolled as the control group....
Results
1.
Comparison of clinical records between the two groups.
There was no statistical difference in age,
gender,
BMI and physical performance between the two groups (Table 1).
The two groups were completely matched in tumor site,
pathological type and tumor stage,
and had no statistically significant difference in metastasis (Table 2).
Stomach (25 cases,
43.10%) and colon (20 cases,
34.48%) were the most frequent tumor sites.
Adenocarcinoma was the main pathological type (86.21%).
Most subjects were in advanced stage and many of them had metastasis...
Conclusion
1.
Risk factors for UPE in malignant oncologic patients.
Risk factors for acute VTE in cancer patients included patient related factors (age,
sex,
race,
performance status,
obesity,
prior history of VTE and number of chronic medical comorbidities),
cancer related factors (primary site of cancer,
histology,
tumor stage and time interval from cancer diagnosis) and treatment related factors (hospitalization,
surgery,
indwelling catheters,
chemotherapy,
hormonal and biological therapy,
supportive measures) [14].
Our study suggested elder patient with adenocarcinoma in advanced stage had high risk for UPE,
which...
