Aims and objectives
To highlight the high utility and accuracy of CT (Computed Tompography) in diagnosing bowel obstruction on the one hand,
and
to expose typical scanographic and intraroperative aspects on the other hand.
Methods and materials
80 patients were eligible to participate in a prospective study from January to September 2017.
Mean patient age was 49 (range,
16-76).
It was about 40 women and 40 men.
They presented with abdominal pain,
vomiting,
inability to defecate or pass gas.
All the patients underwent classic abdominal radiography and CT scan with intravenous administration of iodinated contrast material using a 16 slice CT.
Results
Radiology assumed considerable relevance in assisting the therapeutic decision of surgeonS by addressing these questions:
Is the bowel obstructed?
Obstruction is present when distended bowel loops (diameter >2.5 cm for small bowel (SB) loops and >6 cm for large bowel (LB),
from outer wall to outer wall) are seen proximal to normal-caliber or collapsed loops distally.
What is the severity?
In high-grade obstruction,
there is a 50% difference in caliber between the proximal dilated and the distal collapsed bowel.
Also,
obstruction that has been present...
Conclusion
CT is extremely useful for determining the site,
transition point,
cause and complications of bowel obstruction.
It allows a prompt diagnosis to avoid vascular compromise,
which can lead to gangrene,
perforation,
and death.
Emergency surgery can prevent these sequelae and assess the real cause of the bowel obstruction.
Hence,
collaboration between surgeons and radiologists remains a necessity.
