Learning objectives
We aim to ascertain whether a full swallow protocol remains of diagnostic benefit in patients whom have already undergone an oesophagogastroduodenoscopy (OGD).
Background
Originally this study was started to look at modifying the contrast swallow protocol.
It was suggested that a contrast swallow study onlygives additional information on dynamic pathologies such as reflux and dysmotility.
Therefore a modified protocol with less radiation should be considered,
for example by removing imaging of the upper oesophagus if the patient had a normal OGD.
However after analysis of our results we came to a different conclusion.
Findings and procedure details
Method:
Using the local Computerised Radiology Information System (CRIS),
a search for all contrast swallows across the 6 month period between January to June 2017 was carried out.
Within the clinical information of the study request,
those patients who also had an OGD were identified.
The fluoroscopy report of these patients was then analysed to identify pathology.
The fluoroscopic findings were then correlated to OGD findings.

Excluded: 2 patients had incomplete OGDs,
7 had naso-endoscopy only.

Discussion:
43% of the fluoroscopic studies agreed with the OGD.
Of these 19 were normal studies. 10 contrast studies had additional findings to those on OGD and 36 contrast swallows did not correlate with OGD.
A note here is that 27 of these found dysmotility,
something that is not well assessed on OGD (see below).
Within our populations a total of 27 hiatus hernias were identified,
12 cases were present on both studies,
but 15 were present only on one study.
