Aims and objectives
Evaluation of pain perception during,
and after stereotactic vacuum-assisted breast biopsies (VAB).
Methods and materials
For a period of 15 months,
patients undergoing routine stereotactic VAB (9G and 20mm) after injection of local anaesthetic (10ml Lidocaine HCl 10mg/ml) were asked to participate in an online survey which included 9 questions on pain perception during and after the VAB procedure.
The patients received an invitation form which included a digital link to the survey.
The radiographer noted the number of biopsies on the form.
One week after the procedure,
the patients completed the online survey.

Results
105 patients completed the survey.
On average,
8 biopsies (range 2 -18) were taken.
Overall average pain score for the whole procedure of stereotactic VAB including post-procedural follow-up was mild (2.07).
Specified pain scores for different items included in the survey were as follows (Fig. 2):
During the procedure: local anaesthesia (2.70),
incision (0.78),
needle insertion (1.57) and biopsy acquisition (2.20).
After the procedure: immediate post-procedural pain (2.17),
late post-procedural pain in the evening following VAB (3.27),
late post-procedural pain ±24 hours following VAB (2.74),...
Conclusion
Pain perception during and after stereotactic VAB is mild and local anaesthesia is generally sufficient.
During the procedure,
it makes sense to give extra anaesthesia.
Always listen carefully to the patients and act accordingly.
Guidance by the radiographers,
clear explanation and information about the procedure (such as expecting sound,
feeling and aftercare)is very important.
The pain experienced within the first 24 hours after VAB was reported worst.
In a large majority of cases,
a hematoma in the breast was reported.
