Learning objectives
Common and rare etiology of urinary bladder rupture
Key radiological and urological features of urinary bladder rupture
Background
Injuries to the urinary bladder are relatively uncommon due to its location deep in the pelvis,
where the urinary bladder is well protected against external forces.
Incidence is approximately 1.6% of all abdominal trauma [1].
A high level of clinical suspicion and knowledge of relevant anatomy plays crucial role in the diagnosis [2].
Most cases of the urinary bladder trauma can be managed nonoperatively and the correct classification of injury is necessary for proper management.
Anatomy:
The urinary bladder is a hollow muscular organ located...
Findings and procedure details
In recent years we have found several iatrogenic perforationsof the urinary bladder,
mostly after TURB due to bladder tumor.
One representative case of such IBT is presented in Fig 4.
During last 4 years we have identified 10 urinary bladder ruptures after blunt trauma; 6 extraperitoneal and 4 intraperitoneal ruptures.
We are presenting four cases as a demonstration of various kinds of urinary bladder trauma and its imaging:
A case of a female pedestrian injured during a car accident.
She suffered a minor head injury...
Conclusion
CT cystography is the method of choice for evaluating traumatic hematuria and should be included in patients with major pelvic injury or with free fluid in abdominal cavity,
when the urinary bladder rupture is suspected.
Excretory contrast enhanced CT phase have significantly lower sensitivity for urinary bladder rupture,
but may be useful as the first-line examination.
The differentiation of extraperitoneal and intraperitoneal bladder rupture is necessary for proper therapeutic management.
