Learning objectives
Review the TNM 8 classification of melanoma.
Review the indications of 18F FDG PET CT in cutaneous malignant melanoma.
Learn about the 18F FDG PET CT protocol in cutaneous malignant melanoma and what parameters allow to suspect metastasis.
Background
What is PET CT?
Positron emission tomography-computed tomographyis anuclear medicinetechnique which combines apositron emission tomography(PET) scanner and a tomography(CT) scanner,
to acquire images that demonstrate the metabolism of a tissue.
The most used radiopharmaceutical is fludeoxyglucose F-18,
allowing to assessthe tissue uptake of glucose.
What is cutaneous melanoma?
Cutaneous malignant melanoma is the result of uncontrolled proliferation and/or malignant transformation of the melanocytes located in the basal epidermal layer of human skin(1).
Other locations where melanocytes can be found: eyes,
ears,
gastrointestinal tract,
leptomeninges,
oral...
Findings and procedure details
Rol of 18F-FDG PET CT in Cutaneous Melanoma:
The PET-CT 18F-FDG study in melanoma is the most sensitive for the search for distant metastasis(12).
It is applied in early stages with sentinel node invasion and stages III and IV according to the TNM classification 8(11).
Fig 12-14. Their contribution to the management of the patient is related to the assessment of the response to treatment,
re-staging and/or monitoring of the disease.
The 18F-FDG PET-CT is more sensitive than other tracers in the detection of distant...
Conclusion
The 18F-FDG PET-CT is the study of choice for the detection of distant metastasis in cutaneous malignant melanoma,
influencing the therapeutic and prognosis of the patient.
