Testicular trauma is the third most common cause of acute scrotal pain. As the ultrasound (US) is the first-line imaging modality for this condition, the objective of these educational poster is to exhibit common and uncommon sonographic presentations of scrotal trauma, emphasizing the findings that define a conservative management or surgical intervention.

Testicular injuries are uncommon between genitourinary tract’s lesions, given the anatomical position of the testis and their mobility. The most affected group is males between 15 and 40 years and the mechanism of injury can be blunt or penetrating traumas. Blunt trauma is more common and occurs during sport activities, assaults or bicycles and motorbikes accidents. The spectrum of scrotum trauma injuries includes not only the testis, but also the surrounding tissue. In our study, we evaluated a great number of scrotal trauma cases, mostly subacute, demonstrating classic findings of lesions such as hematoma, fractures and ruptures. We also identify extensive and complicated injuries of these conditions, with anatomopathological and surgical correlation, as well as an accidentally detection of a seminoma of the testis, which occurs in 10% of cases. High-resolution ultrasound with Doppler represents the modality of choice for the initial evaluation of scrotal trauma. It provides a noninvasive evaluation that supports the clinical approach and identify associated complications. As the early recognition and therapeutic approach generally ensure good evolution of this condition, preserving the functions of the structures involved, it is important for the radiologist to be well versed with the appearance of each type of injuries along this spectrum.
