Aims and objectives
The peritonectomy associated with HIPEC (Hyperthermic Intraperitoneal Chemotherapy) is an aggressive therapeutic technique with a curative potential of PC (Peritoneal Carcinomatosis). It requires an adequate selection of patients through PCI (peritoneal cancer index).
The importance of CT in the diagnosis of PC is to determine the radiological PCI to identify which patients will benefit from radical surgery-HIPEC.
Our objective is to determine the radiological PCI and evaluate the degree of concordance with the surgical PCI describing the most frequent causes of discordance.
Methods and materials
An observational,
descriptive and retrospective study of CT with intravenous contrast and portal phase of patients operated on for peritoneal carcinomatosis was carried out from May 2014 to May 2017.
They are assessed by the same radiologist who is part of the unit and who does not know the surgical findings.
The radiological assessment follows the same principles of scoring by quadrants and tumor size that is done in the operating room.
We analyzed the radiological PCI according to the CT and correlated with the...

DEFINITION OF PERITONEAL CARCINOMATOSIS:
Peritoneal carcinomatosis is any tumor spread that affects the serosa of the peritoneum and the intraperitoneal organs in a localized or massive way.
Classically,
this disease has been considered an advanced and incurable tumor stage,
treated palliatively and with fatal outcome.
Since the 1980s,
different therapeutic proposals have emerged for these patients,
with intraperitoneal and hyperthermic chemotherapy (HIPEC) offering the most favorable results to date.
VIEWS OF DISSEMINATION OF PERITONEAL CARCINOMATOSIS AS A METASTATIC DISEASE:
- Hematogenous route: main form of...
Conclusion
- Radiological PCI by means of CT prior to surgery is the most important prognostic tool,
providing a score that determines the therapeutic strategy.
- The agreement between the radiological and surgical PCI requires a radiologist trained in this pathology
- The radiological / surgical PCI correlates significantly in our series in a global way.
- In the analysis by subgroups there is concordance in cases of solid tumors and in low PCI.
- There is no agreement in patients with small bowel involvement,
non-solid...
