Learning objectives
.
Describe the role of radiologic imaging assessment in the evaluation of pyelonephritis.
.
Identify imaging features of pyelonephritis and its associated complications.
Background
Urinary tract infection is the most common urologic disease in the world,
and frequently it is diagnosed by clinical and laboratory findings.
Although radiologic imaging evaluation is not necessary for diagnosis and treatment of uncomplicated cases in adults,
it represents an important tool for the assessment of patients who do not respond to therapy and for those whose clinical presentation is either atypical or potentially life-threatening.
So,
early diagnosis of pyelonephritis is essential to reduce the risk of acute renal failure,
sepsis,
and death.

Findings and procedure details
ACUTE PYELONEPHRITIS
Case 01 - A 27-year-old woman with dysuria,
fever and continuous right flank pain two days ago (Fig. 1).
Uncomplicated acute pyelonephritis is a bacterial infection of the renal pelvis and parenchyma.
Clinical presentation is,
in the vast majority of cases,
related to urinary complaints,
fairly specific and classical in most cases,
consisting of abrupt onset of fever,
flank pain and chills.
The incidence of acute pyelonephritis is approximately five times more frequent in females with an important increase in puberty.
Imaging features...
Conclusion
Although the pyelonephritis-related imagingfeatures may have several patterns,
radiologists must become familiar with key imaging findings seen in those conditions.
