Transverse vaginal septum is a type of rare congenital uterovaginal anomaly (class II under the Rock and Adam classification). The incidence of transverse vaginal septum is 1 in 70,000 female. It may be an isolated anomaly or associated with other Mullerian duct anomalies. It may occur at: upper vagina in 46%, mid vagina in 40% and lower vagina in 14%. This isa prospective study done at a tertiary care teaching hospital over a period of 18 months i.e. January 2017 to June 2018, after the approval from Ethical review committee of hospital. The data was kept confedential. All those patients who had a suspecion of transverse vaginal septum on bimanual gynaecological and speculoscopic examination were included in the study after informed consent. Out of 15 patients who underwent MRI with clinical suspecion of transverse vaginal septum only 7 patients met the inclusion criteria as no signs of obstruction were found on initial MRI of these patients. Rest of the 8 patients who were excluded from the study had obstructed septum with associated hematocolpos/hematometrocolpos and of these 8 patients had OHVIRA syndrome. MRI is a very useful tool to evaluate the vagina and provide essential data for the diagnosis and surgical planning of transverse vaginal septum. Thus radiologists must be familiar with the scan protocols and with the data that must be reported for appropriate clinical decision making. Administration of vaginal gel before pelvic MRI for vaginal septum is inexpensive and well tolerated technique and can improve the diagnosic usefullness of MRI for vaginal septum, which is one of the rare causes of female infertility.
