Aims and objectives
To determine the sensitivity and specificity of 18F FDG PET CT in diagnosis of fever of unknown origin (FUO).
FUO is a diagostically challenging condition and defined as fever greater than 38.3 degree on several occasions for longer than 03 weeks for which the underlying etiology cannot be diagnosed even after at least 01 week of hospital stay.
(1)
The three major etiology would be infection,
malignancy and non infectious infilammtory conditions.
FDG has the advantage that,
it accumulate in infectious / infilammatory and malignant...
Methods and materials
This was a prospective study carried out from October 2017 to December 2018.
31 patients with fever of unknown origin were evaluated with 18F-FDG PET scan.
In our public sector hospital,
this expansive modality was carried out free of cost.
PET-CT images were obtained with Toshiba PET/CT scanner after 55 minutes delay of tracer uptake,
following intravenous injection of18F-FDG.
Plain CT Scan done for anatomic correlation and attenuation correction.
Data analysis was done on SPSS version 22,
Sensitivity,
Specificity,
positive pridictive value,
Negative pridictive value...
Results
Total 31 patients were evaluated with PET scan for the diagnosis of fever of unknown origin.
16 were males and 15 were females with age ranges between 18 to 72 years with mean age 38 years.
Out of 31,
25 were PET scan positive, 24 were true positive and accurately contribute in diagnosis while 01 patient was false positive. 06 patients were PET negative,
out of which 03 were true negative and 03 were false negative.
PET/CT showed sensitivity and specificity 88.8% and 75% respectively....
Conclusion
18F FDG PET CT is preferable sensitive imaging technology in which the positive findings are helpful in conforming the suspected site of pathology or identify the unexpected source of fever either infective/ infilammatory or neoplastic and negative findings exclude suspected source of patholology.
In addition positive findings are also helpful in selecting the approachable site and mode of biopsy for further patient's management.
