Aims and objectives
Breast cancer has become the most common of all cancers and is the leading cause of cancer deaths in women worldwide,
accounting for >1.6% of deaths.
In India,
breast cancer is set to overtake cervical cancer as the most common cancer among women by 2020.[1] For successful management of breast cancer,
earlyand accuratedetection is very important.
Mammography and conventional ultrasound (US) are the main imaging modalities which have shown the highest sensitivity in detecting breast cancer.

Methods and materials
This study was carried out at a tertiary cancer care center in SouthIndia between July 2013 to June 2014 after approval by the Institutional Review Board and Ethics committee.
Patients:
Inclusion criteria:
Consecutive patients with solid/solid-cystic breast masses
Size>5mm
BIRADS 3 or 4 lesions,
which were biopsied after ARFI elastography
Exclusion criteria:
Simple cyst
BIRADS 1,
2 or 5 lesions
Calcified lesions with non-calcified area <1cm
Prior biopsy/FNAC/Surgery/radiation on the same breast

Results
148 breastmasses were included from 113 patients with mean age of 45 years (age range 15-80 years).
Distribution of benign and malignant breast lesions in various age groups are presented in fig.2 & 3.
Mean age for malignant lesions in the studypopulation was 49 years and for benign lesions,
it was 40 years (fig 4).
Of all the benign lesions (74),
fibroadenoma was the commonest (50 lesions,
67%).
Others were fibrocystic changes (7),
epithelial hyperplasia,
granulomas,
benign phyllodes,
lipoma,
papilloma & apocrine metaplasia.

Conclusion
In this study,
we found thatshear waveelastography improves the specificificity of breast US as the shear wave velocity of malignant breast lesionwas significantly higher than the benign lesion.
By adding consideration of mass softness,
as detrmined by quantitative value ofSW elastographycould reduce unnecessary biopsies of low-suspicion BI-RADS category 4 masses.
Stiffness on SW elastographic images also helped in identifying the new malignancies among BI-RADS category 3 lesions which would have been otherwise missed.
However,
further larger prospective validation is required.
