Aims and objectives
Inferiorvena cava filters (IVCF) are devices inserted into the inferior vena cava to interrupt the flow of embolic material from a distal location to the central venous system.
Although they have been used for more than 45 years,
there is still controversy regarding their indications,
especially regarding their prophylactic use.
In our institution they have been used for more than 10 years with different prophylactic indications,
obtaining favorable results for patients.
The present work gathers the experience of the prophylactic use of IVCFin our institution...
Methods and materials
It is a retrospective,
observational,
descriptive study.
A convenience sampling was carried out includingall the patients who had an IVCF insertion in the University Hospital of the Santa Fe Foundation of Bogotá between 2008 and 2016,
who also had a record of their clinical evolution in the electronic system of the hospital.
Once patients were selected,
the procedure records and clinical evolution were reviewed in the digital system of the department and the institution.
Variables were registered in a validated clinical research format which was...
Results
Of a total of 218 patients who underwent IVCF insertionduring the selected years,
133 patients hada prophylactic indication (Figure 2).
Of these patients,
in the majority,
corresponding to 60.1% (80 patients),
it was placed as part of the comprehensive management protocol in patients undergoing cytoreductive surgery combined with intraperitoneal chemotherapy with hyperthermia (Sugarbaker).
The majority of these patients (47.5%) had surgery indication for secondary neoplastic peritoneal compromise with primary gastrointestinal or genitourinary tract neoplasms.
In the other patients,
without secondary peritoneal compromise,
the indication for...
Conclusion
Despite the widespread use of IVCF,
its prophylactic indications are still controversial and are determined mainlyby each institution and its working groups.
In our institution,
most prophylactic IVCFare indicated as part of comprehensive management in patients undergoing Sugarbaker surgery,
which is widely performed as part of the treatment of peritoneal neoplasms.
It is an indication unfrequently mentionedin the available literature,
but being high-risk patients due to their neoplastic compromisewho undergo amajor surgery,
they benefit greatly from the use of filters in their integralmanagement.
In our...
