Aims and objectives
Renal masses have become a common radiologic finding in consequence of the widespread use of advanced cross-sectional imaging.
Some renal lesions are typically benign,
such as simple cysts and angiomyolipomas.
Other solid masses or complex cysts,
however,
must be considered malignant unless proven otherwise.
For the solid cortical renal masses,
histological findings different from renal cell carcinomas (RCCs) may surprise the medical team.
Unusual cortical renal lesions have a wide range of imaging features,
so the knowledge of them is important for radiologists [1–3].

Methods and materials
In this retrospective study,
patients submitted to nephron-sparing surgery (NSS) from January 2011 to December 2017followedon Department of Urology,
University of Sao Paulo Medical School General Hospital,
were analyzed.
In total,
308 patients underwent surgical procedure due to suspicion of malignant primary renal tumor.
Cases of renal cell carcinoma (250 of them) and oncocytoma (32 of them) were excluded.
Patients without computed tomography (CT) or magnetic resonance (MR) images on our database previous to the surgery (18 of them) were also excluded.
Results
In total,
8 patients (mean age of 55.1 years,
range 33-68) with unusual renal masses were found.
Among these patients,
2 had a histopathologic diagnosis of leiomyoma,
one of cystic nephroma,
one of a mesenchymal neoplasm,
one of metanephric adenoma,
one of focal xanthogranulomatous pyelonephritis,
one of breast metastasis and one of an epidermoid cyst.

Leiomyoma is a benign mesenchymal neoplasm originated from smooth muscle cells and in the genitourinary system,
it is more commonly known to affect the bladder.

Conclusion
Cross-sectional imaging of unusual renal masses in adults are varied,
encompassing cystic and solid lesions.
Many diseases can mimic a malignant primary renal tumor and leiomyoma was the most common one in this cohort.
