Aims and objectives
Our aim was to evaluate the evidence for the diagnostic efficacy of 0,3-T MRI for knee injuries using arthroscopy as the reference standard.
Over the years,
with the evolution of machines and study protocols,
MR has been confirmed as a non-invasive and highly sensitive instrument in the evaluation of osteo-ligamentous structures,
articular surfaces and peri-articular knee tissue.
Arthroscopy,
on the opposite side,
confirms a highly sensitive and specific procedure for evaluating endocapsular structures,
but invasive and sometimes not well accepted as diagnostic mode.

Methods and materials
From January 2013 to March 2017 we enrolled 214 patients (143 males and 71 femalesaged from 17 to 72): in 95 patients the study was aimed to evaluating the right knee and in 119 the left knee.
Inclusion criteria were as follows: patients who 1) were subjected to RM knee exam with O-scan equipment (Esaote,
Genoa) with dedicated coil and 2) had undergone diagnostic arthroscopy within 3 months after the examination of RM.

Results
Arthroscopy found:
- 155 medial meniscal lesions;
- 53 lateral meniscal lesions;
- 42 anterior cruciate ligament lesions;
- 3 posterior cruciate ligament lesions;
- 242 cartilage lesions of which 30 patellar,
70 tibial and 142 femoral-trochlear.
The first reader recognized:
- 194 meniscal lesions,
misunderstanding 14 of them (9 of lateral meniscus and 5 of medial meniscus);
- 41 cruciate ligaments lesions,
misunderstanding 4 of them (3 of the anterior cruciate ligament and 1 of the posterior cruciate ligament);
- 136 cartilage injuries,

Conclusion
Our study shows a good accuracy (arithmetic average between the two readers) of the low field in identifying the menisci findings (93%) and cruciate ligaments findings (96%),
while the accuracy of the articular cartilages analysis was less accurate (85%).
Concordance was found to be GOOD for the evaluation of menisci and EXCELLENT for the evaluation of cruciate ligaments (Cohen's Kappa of 0.768 and 0.832 respectively),
