Learning objectives
The purpose of this educational exhibit is to:
1.
Describe and illustrate the MDCT imaging findings of kidney parenchymal changes in patients with alcohol surrogate abuses;
2.
Provide relevant data on the pathomorphology and clinical manifestations in this patients;
3.
Describe the use of contrast enhancedmultiphase MDCT techniques in the differentiation of chronic toxic changes in renal parenchyma with other conditions accompanied by atrophic changes;
4.
Present our experience in studying such patients in past 4 years.
Background
Consumption of surrogate alcohols (i.e.,
nonbeverage alcohols and illegally produced alcohols) was shown to impact on different causes of death,
not only poisoning or liver disease,
and appears to be a major public health problem in Russia,
Eastern Europe's countries and elsewhere.
Surrogate alcohol - different types of liquids,
which are not intended for internal use,
but,
nevertheless,
is used instead of alcohol.
There are many other toxic substances in surrogate alcohol besides ethanol,
such as hydrogen peroxide,
antiseptics,
ketones,as well as alcohols other than...
Findings and procedure details
Twenty two adults (mean age 45.2 years,
range 37.2-64.1years) with same anamnesis,
clinical laboratory and ultrasound data underwent multidetector contrast enchanced CT in our department between November 2012 and September 2016.
All examinations were performed with a 16-slice MDCT imaging system (Siemens Somatom™ Emotion 16; Siemens Healthcare,
Germany) with a standard contrast dose intravenously injected by Medrad™ injector.
In thirteen of twenty two cases CT pattern was quite similar: marked cortical thinning and irregularity (arrows)in combination with normal renal parenchyma on both sides.
Conclusion
Multiphase MDCT is a powerful diagnostic tool in direct accessing the amount of damage of kidney parenchyma in patients with renal failure caused by repeated episodes of drinking alcohol surrogates history and arise as an additional evident factor in the prognosis of the disease in these patients,
which is very important for clinicians.
