Aims and objectives
To evaluate the role of radiomics in preoperative triple-phase computed tomography (CT) to create a predictive score of recurrence in patients undergone orthotopic liver transplantation (OLT) for hepatocellular carcinoma (HCC).
Methods and materials
Out of 202 patients who underwent OLT for HCC in our Centre between 2011 and 2015,
we retrospectively selected 8 (4%) patients (8 males,
mean age 61 years,
total of 20 lesions,
Group 1 Table 1) who had HCC recurrence within two years after OLT and underwent triple-phase CT within 3 months prior to treatment (Lightspeed Pro16 or Optima 660 MG40,

Three different radiomic signatures were developed.
The first signature (λ=λmin) used the minimum λ value and was based on 15 features (Table 3).
The second signature (λ=1 SE) was calculated with a λ value at one standard error from λmin and was based on 9 features (Table 4).
The third signature (λ=1 SE mod) was derived from the second signature by removing shape features and features from portal venous phase,
therefore considering only 6 features from delayed phase (Table 5).

In addition to the small sample number,
the main limits of the study are the lack of the inter- and intra-operator variability analysis in the identification and segmentation of the lesions and the lack of evaluation of the effect of the preprocessing filters.
the diagnostic performances of these rad-scores should be tested on other populations,
in order to prove their efficacy.
