Aims and objectives
Breast Imaging-Reporting and Data System (BI-RADS) 3,
or “ probably benign” lesions are challenging with high inter-observer variation.
The accepted probability of malignancy of these lesions is within 2%.
However,
real-practice incidence of malignancy of BI-RAD 3 lesions is still under investigation.
It is known that the main role for mammography is the early detection of breast cancer in asymptomatic patients.

Methods and materials
Data collection and population.
This retrospective study was compliant with the “CMN 20 de noviembre ISSSTE” institutional review board.
Informed consent was not required.
All consecutive mammography and breast US reports obtained between January 1,
2015,
and December 31,
2017,
were identified in the institutional database.
All breast examinations with short-term follow-up (BI-RADS category 3) were included in the study.
Biopsy for both palpable and nonpalpable masses (BI-RADS categories 4 and 5) as well as masses known to be malignant (BI-RADS category 6) were not...
Results
The study included a total of 591 patients with BI-RADS lesions.
Among them,
322 (55%) continued with short-term follow,
as recommended.
Total population included 321 (99.7%)women and 1 man.
The median age of the cohort was 54 years (interquartile range [IQR],
48-62 years).
At a median time of 8 months (IQR,
6-13 months) imaging evaluation that included mammography and ultrasound was performed.
In total,
242 (68%) were re classified as BI-RADS 2,
80 (22%) persisted with BI-RADS 3,
and 37 (10%) were classified as BI-RADS...
Conclusion
Among patients with BI-RADS 3 lesions,
the incidence of malignancy can reach 4%.
Personal history of breast cancer was found as a predictor of malignant outcome.
Further research is required to asses the impact of personal history during BI-RADS classification,
especially for BI-RADS 3 lesions.
Early detection of malignancy in patients with low probability of breast cancer,
such as BI-RADS 3 lesions,
is key for any public health program.
