- Compare BIA-ALCL (Breast Implant Associated Anaplastic Large Cell Lymphoma) findings described in literature with SIGBIC (Silicone-Induced Granuloma of Breast Implant Capsule) findings described by our group. - Determine diagnostic criteria for diagnosis. - Discuss similarities between the two pathologies.

BIA-ALCL has been subject of studies in the last years, especially after the increase of disease reports in the last 3 years. (1-6) Currently, less than 600 cases are described in international literature. Recent studies tried to link the BIA-ALCL development with the presence of silicone breast implants, however there is no consensus regarding the origin of the disease. (6-8) Some studies attempt to correlate with bacteria forming biofilm (infectious processes- Ralstonia species) or the textured surface of breast implants that would cause fibrous capsule...

Patients with clinical complaints related to breast implants were submitted to diagnostic breast MRI with the use of intravenous contrast. Patients with SIGBIC findings were referred for percutaneous ultrasound guided biopsy / surgical capsulectomy. The findings found in our study were compared with BIA-ALCL findings described in the literature, most of them published as case reports or reviews. Our findings indicate that differential diagnosis of ALCL and SIGBIC by imaging is limited. Diagnostic should be made by the presence of anaplastic monoclonal T cells under optical microscopy, enhanced by immunochemistry CD30+ test. Our hypothesis is that these pathologies may be part of the same spectrum of disease due to immune reaction to the silicone gel bleeding/ leakage.
