Learning objectives
-Learning the different radiologic patterns of CNS involvement in behçet disease.
- Recognizing the MR features of the different patterns of neuro-beçet disease,
and the differential diagnosis for these features.
-The role of the new MR techniques in the diagnosis of CNS involvement in behçet disease.
Background
Neuro-Behçet’s disease (NBD) is the neurological involvement in Behçet’s disease (BD).
It occurs in 5 to 30% of patients with BD.
It could be either acute or chronic and considered one of the most devastating manifestations of the disease owing to its high morbidities that can lead to permanent deficits or death.
The diagnosis of neurobehcet disease becomes a real challenge when the neurological symptoms are the first presentation of the disease which is the case in 5-10% of patients,
so we need to consider...
Findings and procedure details
Neurobehçet disease (NBD) could be categorized into three main categories; parenchymal,
extraparenchymal and mixed parenchymal and non-parenchymal disease.
Atypical parenchymal CNS involvement forms are also reported.
Parenchymal NBD:
it is the most common form of CNS involvement in NBD in adults, also known as the intra- axial NBD,
it represents an inflammatory meningo-encephalitic process,
that usually involves the telencephalic-diencephalic junction,
brain stem and spinal cord.
These patients present with a subacute (or rarely acute) onset of severe headache,
dysarthria,
hemiparesis,
ataxia and cranial nerve palsy.

Conclusion
MRI has a major role in confirming the diagnosis of neurobehçet disease with its parenchymal and extraparenchymal forms.
