Purpose
To evaluate the diagnostic performance of digital mammography (MMG), contrast spectral dual-energy mammography (CESM) and (99m) Tc MIBI scintimammography in detecting multicentric breast cancer.
Methods and materials
437 women were examined at N. N. Petrov National Medical Research Center of Oncology; the mean age of the women was 50 years.
Mammographic digital images were obtained on Senographe DS «GE» unit with a flat panel detector (Se, 100 μm) and image size - 24х31 cm² = 7.4 megapixel. Mammographic examination was performed with breast compression and included routine craniocaudal (CC) and mediolateral oblique (MLO) views of the breasts. Results
367 patients were diagnosed with breast cancer. 320 (87%) had unifocal lesions and 47 (13%) had multicentric lesions. CESM was performed in 112 women, 35 of them with multicentric breast cancer.
Sensitivity, specificity and accuracy in the diagnosis of the multicentric breast cancer were 70.6%, 73.1%, 72.3% for digital mammography and 83%, 98%, 96% for scintimammography, respectively (Fig. 5.).

Conclusion
CESM and (99m) Tc MIBI scintimammography showed high sensitivity, specificity and accuracy rates in multicentric breast cancer detection, as was evidenced by a high percentage of coincidence of pathological and radiological findings. The use of CESM and (99m) Tc MIBI scintimammography can significantly increase detection rate of multicentric breast cancer in comparison with digital mammography, primarily in women with high breast density.
