Purpose
Mammography is a low-dose x-ray procedure that allows visualization of the internal structure of the breast. The addition of breast tomosynthesis (TS, 3D mammography) to digital mammography reduce false positives and improve cancer detection compared to digital mammography alone. Therefore, screening mammography (SM) is the most effective tool to find breast cancer (BC), which vary by age.

Methods and materials
An observational study performed during the period between January 2015 and May 2019 included 17 women under age 40 (age range 20-40 years; average age 30) and 17women over age 70 (age range 70-88 years; average 79), who underwent VABB for suspicious microcalcifications with a Digital Breast Tomosynthesis–guided Vacuum assisted Breast Biopsy (DBT-VABB). Results
Of 427 biopsies:
- 17 patients under age 40 (representing 3.9%) were performed DBT-VABB in order of frequency amorphous (12, 70.6%) (Fig 3, 4, 5), fine pleomorphic/linear (3,17.6%) (Fig 6, 7) and coarse heterogeneous (2, 11.8%) (Fig 8) microcalcifications. Histopathological results were 7 Atypical Ductal Hyperplasia (ADH), 4 sclerosing adenosis, 4 fibrocysticbreast disease (FBD), 3 Typical Lobular Hyperplasia (TLH), 2 Infiltrating Ductal Carcinoma (IDC), and 1 Ductal Carcinoma In Situ (DCIS).

Conclusion
Our results suggest that in both groups of ages were found most frequently clustered microcalcifications, and the main malignant pathology was IDC. As differences between them, in women under age 40 were found amorphous microcalcifications with bening results and high-risk breast lesions (these data differ from published studies) (10); and in women over age 70 were found amorphous and coarse heterogeneous microcalcifications with mainly malignant results and less number of high-risk breast lesions.
