Background
Acute pelvic pain in women is a nonspecific symptom that is generally defined as pain of the lower abdomen or pelvis that has lasted less than three months. Most acute pelvic pain is caused by reproductive, urinary or gastrointestinal tract disorders, although there are musculoskeletal, vascular, or neurologic processes that can contribute as well. This work is focused on gynecologic conditions.

Functional cysts
The most common cystic ovarian lesions are benign functional ovarian cysts. Pain may develop because of rapid cystic enlargement, rupture, or hemorrhage, alone or in combination.
-Follicular cysts are the most common well-defined adnexal masses(2). Most of them are asymptomatic and incidentally discovered in cases of acute pelvic pain( Fig. 3 , Fig. 4 ).In these cases, radiologists often do not know whether follow-up is clinically indicated. Conclusion
1.Gynecological pathology must be present in the differential diagnosis of acute pelvic pain in women.
2.The differential diagnosis of pelvic pain is most logically divided by the age and pregnancy status of the patient.
3.Ultrasound is the main method of diagnostic evaluation, but it has limitations. CT is relegated to doubtful cases and to assess the extent of the process or possible complications.
4.Images need to be correlated with the patient’s clinical history and laboratory values ( Fig. 31 , Fig. 32 ).
