Learning objectives
Describe the technic and the various ways of exploring by ultrasound, the neonatal head.
Present common anatomical findings that can be foundin preterm and term newborns, as well as anatomical variants.
Perform a revision of the different brain pathologies whichaffect newborns, specially the preterm ones, such asgerminal matrix hemorrhage,periventricular leukomalacia andhydrocephalus.
Background
The neonatal brain ultrasound has an important role in the evaluation of the newborn, especially in high risk preterm ones. It is a safe, low cost, harmless method without radiation, that doesn´t need sedation of the patient and let us examine critically ill patients without moving them.
The newborn skull can be penetrated with ultrasound using the fontanels as acoustic windows.Fig. 1
Findings and procedure details
1.TECHNIC
Principally, the brain can be explored by the anterior fontanel, in B mode on gray scale, including views in coronal, sagittal and other planes, depending of the patient pathology.
There are other skull windows that we can use, for example the posterior and mastoid fontanels, for the evaluation of the posterior fossa and the occipital hornsof the lateral ventricles.
The temporal window allows us to see the mesencephalon and the brainstem, and the coronal and squamous sutures let us visualize the brain convexity looking...
Conclusion
Ultrasound is the ideal diagnostic method for the evaluation of the premature neonatal brain, because it is harmless, economic and low rate of colateral effects, compare to other diagnostic alternatives such as tomography. Moreover, it provides great information about the neurologic state of the patients.
