Learning objectives
To review the imaging characteristics of gynecological devices (Intrauterine (IUD) and Intratubal Devices (ITD), and sling implants for incontinence) on three-dimensional Ultrasound (3DUS), regarding normal and abnormal positioning, as well as potential complications and associated pathologies.
Background
3DUS has become a problem-solving technique in the evaluation of a variety of gynecologic disorders, and the imaging of gynecologic devices is one of the indications for this technique. It can add significant clinical information to that obtained by two-dimensional ultrasound (2DUS). Unlike MRI and CT, 3DUS has the advantage of being rapidly accessible, without the use of radiation as well as a low cost.

For the study of IUD and ITD, sonographic acquisition can be obtained via a transabdominal or transvaginal approach, however, the transvaginal approach has become the most useful for reconstruction of the coronal plane with 3DUS. The operator holds the transducer in place while the probe sweeps across the obtained volume with images being electronically stored. 3D images are then processed and modified, and can then be displayed in multiple planes and orientations, such as sagittal, coronal,...
Conclusion
The radiologist should recognize the normal appearance of gynecological devices and associated complications, using 3DUS as a problem-solving technique that improves the diagnostic ability of 2DUS. Computed Tomography and MRI are limited as first line modalities for the evaluation of gynecological devices due to the use of radiation and elevated costs, respectively. Urogynecology is one area where 3DUS is successful enough to replace previous imaging modalities.
