-To present an overview of radiomics in the field of pancreatic neoplasms and discuss its main limitations.
Background
Imaging plays a pivotal role in the diagnosis and management of pancreatic neoplasms,but there are still some open challenges(Table1).
Pancreatic ductal adenocarcinoma(PDAC)
PDAC is an aggressive cancer with a 5-year overall survival rate of around 5%[1].CT is the method of choice for the diagnosis and staging as it allows an accurate detection of the disease (sensitivity 90%). Ten percent of patients, though, present with iso-dense PDACs which are not recognizable on routine CT-scans[1].Although MRI has a sensitivity of 80% in this setting[1], some patients still...

Chu et al.[7] evaluated the role of radiomics features(RFs) in differentiating between normal pancreatic parenchyma and PDAC-containing pancreatic parenchyma and found that the most informative RFs were able to achieve an accuracy of 99.2%. Although mean tumor size(4.1 cm) was larger than that of PDACs usually missed by radiologists,this study provides initial evidence of the potential ability of radiomics to improve early PDAC detection.

Conclusion
Radiomics may allow early identification of PDACs and response to therapy, and the non-invasive characterization of PCNs and prediction of their evolution to malignant lesions.Nonetheless, several key problems need to be solved before this form of precision imaging can enter the clinical environment.Image acquisition standardization and features definition and extraction harmonization are only a few examples of those key issues.Well-planned prospective and multicenter studies using international standards and guidelines(e.g.International Biomarkers Standard Initiative) will be required to further validate the results found so far.
