Purpose
When clinicians encounter patients with an ovarian tumor, a correct presurgical diagnosis is essential because management of the lesion will vary according to its nature (benign, borderline, or malignant). In this respect, malignant adnexal tumors, which have a high mortality rate1, are the main indication for gynecologic oncological surgery2. Surgical treatment for malignant adnexal tumors is more aggressive than that for other tumor types3.
The first test indicated for evaluation of a pelvic tumor is ultrasound; however, this imaging technique is sometimes inconclusive. The European...
Methods and materials
Patient population
We retrospectively reviewed the pelvic MRI scans of patients of legal age (over 18 years old) who underwent imaging examinations of an undetermined pelvic tumor in our center. Patients in whom no adnexal tumor was identified on MRI were excluded. The study population comprised patients with a definitive diagnosis obtained either by anatomopathological results of a surgical specimen or by evidence of stability of the lesion on imaging examinations for at least 1 year.
Data acquisition
The MRI studies were performed with a...
Results
We evaluated the data of 40 patients who underwent MRI studies from November 2016 to October 2018. The definitive diagnoses of the lesions among these 40 patients comprised 20 benign tumors, 4 borderline tumors, and 16 malignant tumors (Figure 2).
Curve type analysis
Among the 40 patients, we obtained 4 type 1 curves, 22 type 2 curves, 13 type 3 curves, and 1 type 4 curve. We combined types 1 and 2 as Group A (n = 26) and types 3 and 4 as Group...
Conclusion
The main conclusions of the study are as follows:
- MRI is the test of choice for the study of ultrasonographically indeterminate ovarian tumors.
- Including a dynamic MRI sequence can help in the differential diagnosis of the tumor type, especially when other sequences are inconclusive. This can occur when differentiating between borderline and malignant lesions; such differentiation is very important when choosing the surgical approach to the lesion.
- The parameters of dynamic MRI sequencing that are useful for identification of malignant lesions are...
