Learning objectives
1. Utilization of bariatric surgery has been steadily increasing since the early 2000s. As more surgeries are performed, radiologists should become familiar with complications that arise acutely and remotely from the initial surgery.
2. Many complications of bariatric surgery are best demonstrated fluoroscopically. Understanding fluoroscopic imaging appearance is important in caring for this patient population.
Background
Bariatric surgery has become a popular surgical approach to weight loss. Gastric bypass and sleeve gastrectomy are two common bariatric surgeries performed. Gastric bypass surgeries involve dividing the stomach into two portions: a small pouch, which restricts food intake, and a bypassed stomach. The stomach is first cut into long-axis to form a small pouch (Figure 1B) with a volume typically < 30 mL, which remains connected to the esophagus [1]. The proximal jejunum is also cut (Figure 1B), and the proximal 75-150 cm of...

Case 1: 24-year-old female with a history of Roux-en-Y gastric bypass surgery presenting with a perforated marginal ulcer (Figure 6).
Case 2: 51-year-old female with a history of Roux-en-Y gastric bypass surgery presenting with an anastomotic ulcer (Figure 7).
Case 3: 51-year-old male with a history of Roux-en-Y gastric bypass surgery presenting with an anastomotic leak (Figure 8).
Case 4: 46-year-old female with a history of a Roux-en-Y gastric bypass surgery presenting with...
Conclusion
Bariatric surgeries are a popular form of weight management but may have serious complications, including leaks, fistulas, abscess formation, marginal ulcers, and bowel obstructions. Imaging plays a key role in the prompt diagnosis and management of these complications.
