Aims and objectives
- Tumorous pathology of the pituitary region is rich and diverse.
- It mainly consists of macro and microadenomas,
which represent:
o 15% of all the intracranial tumors.
o 90% of the pituitary tumors.
- It mainly affects adults ++,
and is exceptional in children.
- The aspect od pituitary adenomas varies in relation with their volume and their secretory type.
- Pituitary adenomas are classified according to:
Anatomy,
in:
- Pituitary picoadenomas: when the diameter is <3mm.
- Microadenomas: between 3-10mm.
- Macroadenomas: >10mm...
Methods and materials
- Retrospective study: 43 patients.
- Central Service of Radiology,
Ibn Rochd University Hospital of Casablanca,
Morocco.
- One year period: from January 2018 to October 2018.
- Cerebral MRI:
o 1,5 Tesla equipment.
o High resolution imaging.
o Sagittal sections in spin echo in T1 weighting of 3mm thickness.
o Coronal sections in spin echo in T2 weighting,
focused on the hypothalamo-hypophyseal region of 3mm thickness.
o T2 FLAIR.
o Sections with and without gadolinium injection.
o Thin sections (2 to 3mm).
o Dynamic...
Results
Epidemiology:
· Average age: 32 years.
· 28 women/15 men.

- The clinical symptomatology consisted mainly in hormonal and neurological troubles.
- Other means of revelation:
o Intracranial hypertension syndrome.
o Hormonal troubles.
o Vision troubles.
o Hyperprolactinemia.
o Galactorrhea-amenorrhea.
o Primary infertility.
o Acromegalia.
o Epilepsy.
o Cushing syndrome.
Imaging :
INTRASELLAR PATHOLOGY:
Pituitary adenoma is a benign tumor which develops from anterior hypophyseal cells.
v Microadenomas: 18 cases.
- Intra-sellar lesion.
- T1: Iso or hyposignal / normal hypophysis.
- T2:...
Conclusion
- Pathology of the sellar region is diverse.
- MRI is the preferred examination for the diagnosis and follow-up of most of its pathologies.
- It allows a better analysis of the topography,
volume,
relations,
extent and impact of the tumor process.
o Diagnostic approach+++
o Differential diagnosis
o Extension work-up+++
- Upward: opto-chasmatic cisterns,
optic chiasm and 3rd ventricle.
- Onward: frontal lobe.
- Downward: sphenoidal sinus and nasopharynx.
- Laterally: cavernous loggiae,
temporal fossae.
- Backward: cerebellopontine cistern.
