Aims and objectives
Our study was aimed to determine the relationship between radiological findings,
with special focus on tumor heterogeneity assessed by means of diagnostic imaging techniques,
and survival outcomes in patients with synovial sarcoma.
Methods and materials
This was a retrospective cohort study which employed 22 histological proven synovial sarcoma cases done from January 2007 until August 2017 in the radiology department of our institution.
Retrieved information included age at diagnosis,
sex,
clinical presentation,
tumor localization,
tumor size (<= 5 versus > 5cm),
histological subtype (biphasic versus monophasic versus poorly differentiated),
histological tumor grade (according to the Fèdèration Nationale des Centres de Lutte Contre le Cancer (FNCLCC) grading system),inmunohistochemical characteristics and treatment modalities.

Results
Patients ranged in age from 21 to 80 years (mean age,
52 years).
Men and women were affected almost equally (M/F ratio,
1:1,4).
The most common symptom at presentation was a palpable soft-tissue mass (thirteen of twenty two patients).
Four patients reported swelling,
three experienced local pain,
one patient reported pneumothorax,
and one had constitutional syndrome.
The most frequent locations were,
in this order: foot (6/22),
knee (3/22),
thigh (3/22),
armpit (3/22),
neck (2/22),
thorax (2/22),
forearm (2/22) and scapular region (1/22).

Conclusion
Larger tumor size,
the presence of calcifications on CR and MRI and the presence of heterogeneity on MRI were associated with a higher incidence of metastastic disease and poorer prognosis in our study.
The presence of predominantly lung and pleural metastases is characteristic of synovial sarcoma.
Some imaging characteristics of synovial sarcoma,
taken together,
may aid in the diagnosis such as: soft-tissue mass near a joint of extremities; heterogeneity on T2WI; solid post-contrast enhancement; the presence of triple and grape sign…However,
