Aims and objectives
Diseases of the thyroid gland occupy a key place in the structure of endocrine pathology,
palpable nodular goiter become apparent with 2 -5% in the general population.
In the structure of nodular goiter,
malignant tumors account for 1-5% [1].
The frequency of manifestations new cases of nodules is 0.1% per year.
Ultrasound examination (US),
despite the proven high informative value in the differential diagnosis of thyroid masses,
has limitations.
Native US (B-mode) allows to evaluate the morphological signs of the formations,
especially the shape,
contour,...
Methods and materials
The study was performed on HITACHI AVIUS and SUPERSONIC AIXPLORER.
111 patients were included in the study (85 women,
21 men,
mean age 52,3 ± 4,5) with morphologically proven thyroid nodules.
The maximum diameter of the studied lesions was 6–31 mm (mean ± SD,
18 ± 7 mm).
For each site,
standard sonography was lead (B-mode and Doppler techniques),
strain ratio (StR) and strain elastography,
an image evaluation of real-time elastography (RTE) and shear wave elastography (SWE) were performed.
The diagnostic data of the two...
Results
An analysis was made of the results of 111 patients,
which included the B-mode,
the modes of CD + PD,
RTE,
SWE.
They were divided into 2 groups: patients with focal and diffuse variations 56 (49.5%) and only focal variations 55 (50.5%).
46 (44.4%) nodular formations of solid and solid cystic structure 65 (58.6%) were analyzed.
Ultrasound signs of malignancy lesions in the study of gray scale are irregular shape,
vertical orientation,
contour roughness,
hypoechogenicity,
the presence of microcalcifications.
Some help in identifying the malignancy...
Conclusion
The difference StR between benign and malignant nodes is statistically significant (SMD = 2.01,
93% CI [1.62,
2.53],
P <.00001).
The overall sensitivity of RTE and SWE was roughly comparable,
while the difference between the two methods was statistically significant.
In addition,
a statistically significant difference was observed between RTE and SWE (P <.01).
The specificity of RTE was statistically higher than that of SWE; which suggests that,
compared to SWE,
RTE may be more accurate in differentiating benign and malignant thyroid nodules.
Both RTE...
