Purpose
To study the relationship between the presence of edema in the superolateral Hoffa’s fat pad and the presence of patello-femoral structural abnormalities.
To emphasis on the importance of this finding which can be subtle,
overlooked,
missed or ignored.
Methods and Materials
Retrospective case-control study done on 40 cases with positive Hoffa’s fat pad edema on MRI and 39 control cases in Hamad General Hospital,
Qatar.
Both groups analyzed for:
1.
Chondromalacia patellae.
2.
Patellar tendon-patellar length ratio (Insall-Salvati): normal 0.8-1.3.
3.
Ventral Trochlear Prominence: ≤ 3 mm is normal,
> 3 mm is abnormal.
4.
Tibial Tuberosity to Trochlear groove distance N: ≤ 10 mm,
> 10 mm is abnormal.
5.
Trochlear sulcus depth: N: ≥ 3mm,
< 3 mm is abnormal

Results
Chondromalacia was found in 33 cases of positive Hoffa’s fat pad edema (82.5%) and in 7 cases of the control group (17%).

Abnormal measurement of Patellar tendon-patellar length ratio (>1.3; patella alta) was found in 19 cases of positive Hoffa’s fat pad edema (47.5 %) and in 2 cases of the control group (5%).

Conclusion
Superolateral Hoffa's fat pad edema:
Is an important indicator of underlying patellar mal-tracking /dysplasia
May predispose to development of Chondromalacia patellae in young age.
