Purpose
We present two cases of an aberrant left pulmonary artery, also known as pulmonary artery sling,a rare congenital anomaly, diagnosed using thoracic computed tomography angiography.
Congenital heart defects occur in approximately 50% of patients with pulmonary artery sling, most commonly atrial septal defect, ventricular septal defect and patent ductus arteriosus (1)
CT angiography enables detailed evaluation of the suspected anomaly and enables the visualisation of the anatomic relations with surrounding structures. Thus, CT angiography can facilitate the detection of other, commonly associated, congenital anomalies.
Methods and Materials
We evaluated two pediatric patients and performed thoracic CT angiography with iodine nonionic contrast medium at a dose of 1.5 mL/kg by power injection.
The 1st patient, a 2-year-old boy, was examined in 2011 using a 64-slice scanner (Lightspeed VCT, General Electric ). Postprocessing of the data was on GE working station.
The 2nd patient, a 10-months-old girl, was examined in 2018 using a 256-slice scanner (ICT Philips scanner). Postprocessing of the data was on Philips working station Intellispace.
Both cases were characterised by a...
Results
We diagnosed both cases with left pulmonary artery sling (case 1, Fig. 1-2; case 2, Fig. 6-7); the anomaly is characterised by an abnormal origin of the left pulmonary artery, arising from the posterior aspect of the right pulmonary artery that crosses the mediastinum, protrudes between the trachea and oesophagus causing compression of the trachea (case 1, Fig. 5; case 2, Fig. 8) .
Furthermore, in both cases, we detected associated congenital vascular anomalies. In the first case, we confirmed the presence of a persistent...
Conclusion
CT angiography is an excellent non-invasive imaging modality to detect left artery pulmonary sling (1, 2)
The approach enables the diagnosis of associated cardiovascular anomalies.
CT angiography enhances the evaluation of the anatomical relationships of the tracheobronchial tree (7)
Collectively, CT angiography facilitates clinical decision making (2, 7 & 9).
