Results
According to the 18F-FDG PET scans performed early during chemotherapy patients were subdivided into two groups.
PET-positive group consists of 36 (50,7%) patients with persistent abnormal uptake of radiopharmaceutical.
35 (49,3%) patients were considered negative and included in PET-negative group (fig.
2).
Analysis of PET-negative group results showed that 29 (82,9%) patients stayed in CR after a median follow-up of 405 days,
range 78-2292 days.
All patients having continuing complete remission became FDG-negative already after 2-3 cycles of CHT.
In 6 (17,1%) patients of this group,
who had CR on midtreatment PET scans and were negative after 1st line chemotherapy,
relapse was revealed in the long-term follow-up (fig.
3).
Analysis of the results of PET-positive group showed that only 8 patients had a complete metabolic response after 1st line CHT.
In the remaining 28 patients metabolic activity of the disease was persisted.
During further treatment after a median PFS 585 days (range 46-2160) 20 (55,6%) patients maintained the metabolic activity of the disease.
And the majority of them (12 patients) showed disease progression.
In 16 (44,4%) patients further treatment was successful and resulted in a complete remission (fig.
4).
PFS calculated by Kaplan-Meier survival analysis also confirmed the negative tendency in PET-positive group.
Positivity of interim 18F-FDG PET was associated with shorter PFS - median 537 days,
compared with negativity of 18F-FDG PET,
which had a median PFS - 1841 days.
Comparison between groups,
using the log-rank test,
indicated a statistically significant association between PET results and PFS (p=0,0002) (fig.
5).
