Conclusion

- Splenic steal syndrome is an uncommon complication in liver transplants that leads to graft ischemia. Other causes of graft ischemia such as hepatic arterial thrombosis and acute rejection need to be excluded before this diagnosis.

- The findings by the image are essential for the diagnosis. The findings on ultrasound and CT are suggestive of this syndrome, but angiography confirms it by clearly showing preferential flow into the splenic artery.

- Currently, splenic artery embolization is an effective method of reducing splenic arterial flow and restoring hepatic circulation.
