Conclusion
BI-RADS 3 is an intermediate category that appears to be evolving as we learn more about our imaging techniques for detecting breast cancer. While mammographic BI-RADS 3 categories in women over 60 have a likelihood of malignancy of greater than 2%, it may still be appropriate to closely follow-up these patients after a open discussion about risks and benefits. A few findings on ultrasound previously categorized as BI-RADS 3 can now be downgraded to BI-RADS 2 including clustered microcysts, complicated cysts, and multiple bilateral circumscribed masses. BI-RADS 3 should be avoided for MRI, especially in high risk screening studies which are not baseline, as these have a higher likelihood of malignancy.
