Purpose
To evaluate the efficacy of a POC device by comparison with the gold standard laboratory method.
Methods and materials
BACKGROUND:
Intravenous iodinated contrast is a commonly used diagnostic aid to improve image quality and enhance lesion detection on computed tomography. While this is an obvious benefit, there exists a small associated risk of contrast induced acute kidney injury (CI-AKI). CI-AKI is defined as AKI occurring 24-72 hours post administration of IV contrast that cannot be attributed to an alternative cause. One of the biggest risk factors for developing CI-AKI is the presence of pre-existing renal dysfunction, making it essential to exclude renal dysfunction prior...
Results
139 patients were included in the study (mean age 59). Statistical analysis revealed that the StatSensor had a R2 value was 0.77, coefficient variation was 10.65%. 10 patients were of high risk, 26 were of moderate risk and 103 were of low risk based on the POC result. There were no false negatives in the high risk or medium risk group which means no patient with normal POC value had renal impairment based on the laboratory method. However, the false positive rate (low POC result...
Conclusion
The POC device is only moderately accurate at detecting renal impairment in patients undergoing IV contrast. POC would seem to be a good screening tool prior to IV contrast administration, however any low POC eGFR values should be further examined as there could be some false positives.
