Purpose of this study is to describe a case of acute pulmonary edema,
fatal,
occurred in a patient in poor general condition and symptoms for intestinal obstruction,
following accidental aspiration of Gastrografin; review of the literature
Background
Potential adverse effects are known (acute pulmonary edema by aspiration,
anaphylactoid reactions) after administration by mouth hyperosmolar water-soluble iodinated contrast media,
although a few cases in the literature are described.
Gastrografin (BAYER Spa) is a water-soluble radiographic iodinated contrast agent,
high osmolarity.
Containing 660 mg/ml diatrizoate meglumine and 100 mg/ml diatrizoate sodium,
in gastrointestinal solution.
The contraindications are generally reported as usual to iodinated contrast media,
which must be added the special precautions in patients with suspected possibility of aspiration or broncho-oesophageal fistula,
as hyperosmolarity...
Imaging findings OR Procedure details
Comes to our attention a 82 years old male,
with complex medical history and poor physical condition,
ileal anastomosis recently worked for and with previous cholecystectomy and pancreatic resection for hemorrhagic-necrotic pancreatitis.
Abdominal pain,
primarily epigastric,
as well as the recent emergence of vomiting.
Presence of massive sub-umbilical hernia,
abdominal fairly treatable,
without signs of peritoneal irritation.
Physical examination: MV spread,
reduced to the basics.
In the ward are given orally 50 ml of diluted 50% Gastrografin,
with a serious deterioration of already poor clinical...
Conclusion
It 's been experimentally demonstrated,
in rodents,
the irritancy of the respiratory epithelium hypertonic iodinated contrast medium,
which produces pulmonary edema,
when introduced by bronchial.
In 1968,
reported the death of a 60 years old patient,
with cardiopathy and dysphagia,
accusation of respiratory failure after inhalation of Gastrografin.
In 1974 a 78 years old patient,
Boerhaave's syndrome comes to death,
as a result of pulmonary edema,
24 hours after injection of sodium diatrizoate (Hipaque).
