Learning objectives
The objectives of this study are:
To illustrate,
with the help of CT-angiography (CTA),
in what degree the existence of a non-traumatic subarachnoid hemorrhage (SAH) is related to the presence of a cerebral aneurysm.
To evaluate the accuracy of CTA in the diagnostic of cerebral aneurisms,
by usingdigital subtraction angiography (DSA) for confirmation.
To discuss the possible reasons for which none of these two imagistic methods cannot sometimes identify a cerebral aneurism as a cause for SAH.
Background
Subarachnoid hemorrhage represents a specific type of stroke,
in which bleeding occurs in the subarachnoid space (Fig.
1,
Fig.
2),
with an average incidence of 6 per 100.000 annually.
Ethiology
The etiology of SAH can be summarized as following:
A. Traumatic SAH (most frequent case)
B. Spontaneous SAH.
It is estimated that more than 80 % of the non-traumatic SAH are caused by the rupture of a cerebral aneurism.

much less frequent pathologies are involved,

Findings and procedure details
We conducted a retrospective study on a group of 87 patients that were diagnosed with spontaneous subarachnoid hemorrhage (SAH) using unenhanced CT scans,
from November 2013 to October 2014.
In this study we included only the patients to which any recent traumatic history could be ruled out.
For all of these patients,
CT-angiography (CTA) has been performed in order to detect the presence of cerebral aneurism.
For both the unenhanced CT scans and the CTAs,
we used a 16 slice,

Conclusion
The majority of non-traumatic subarachnoid hemorrhage is caused by the rupture of a cerebral aneurysm,
which can be identified in most of the cases using CT-angiography (CTA).
However,
in a small percentage of cases,
digital subtraction angiography (DSA) is required for the visualization of the aneurism.
This implies that CTA alone cannot exclude the presence of an aneurism in a patient with SAH,
since there is still number of cases when the diagnostic can only be established with DSA.
