Learning objectives
To analyze and compare the features of Ectasia of the duct of Wirsung (main pancreatic duct MPD) in various pancreatic tumors based on CT and MRI imaging.
To describe the perls of CT and MRI imaging findings that might be useful in differential diagnostics.
Background
Duct of Wirsung is the main duct of the pancreas,
which is connected to smaller channels.
The physiological function of the duct is to transfer secretion of the pancreas into the digestive tract.
The duct begins at the tail of the organ,
passes through the entire body to its head and opens into the duodenum,
has about 20 cm in length,
up to 4 mm in diameter with small gradient:tail 1.5 mm,
body 2.5 mm,
head 3.5-4.0 mm.
Unexpanded branch ducts are normally not visible...
Findings and procedure details
CT and MRI studies of 46 patients with verified pancreatic tumors accompanied by MPD dilatation were analyzed.
Percentage of observed tumors: ductal adenocarcinoma - 39 (85%),
intraductal papillary mucinous tumor MD - 4 (9%),
intraductal papillary mucinous tumor BD - 1 (2%),
lymphoma - 2 (4%).
The studies were performed on a 64-detector CT scanner and 1.5T MRI +MRCP,
with intravenouscontrast mediausing a protocols according to NCCN Guidelines 2.2016.

Conclusion
Ectasia of Wirsung duct with different morphological types of pancreatic tumors has certain radiological differences,
the analysis of which can help in differential diagnostics.
The presence of the point of obstruction,
abrupt cutoff with upstream dilatation MPD,
the degree of dilatation was significant even for tumors of small,
notexpanded brunch-ducts,water-like fluidcontent in duct - are highlytypically radiologiccriteria for ductal pancreatic adenocarcinoma.
Extremely,
fusiform duct expansion increased to larger sizes without gradient (up 35 mm),
without the point of obstruction,
duodenal papilla with protrusion,
grape-like dilatation...
