Learning objectives
The aim of this study is to discuss the role of computed tomography (CT) imaging in the assessment of living renal donors,
emphasizing the checkpoints for a structured report.
Background
The last decades have shown a significant rise in the number of patients affected by chronic kidney disease (CKD).
The estimated incidence rate of 7% per year in developed countries can be explained by the aging population and the global epidemic of type 2 diabetes mellitus.
Given the progressive nature of CKD,
there has been an increase in the number of patients affected by end-stage renal disease (ESRD),
consequently magnifying the demand for replacement therapies such as dialysis and transplantation.

Findings and procedure details
1 PROTOCOL
Potential living renal donors are generally healthy and young individuals,
therefore efforts should be made to minimize radiation dose while maintaining accuracy,
as proposed by the ALARA (as low as reasonably achievable) principle.
The protocol currently used at our institution is based on the split bolus technique.
An unenhanced phase is obtained to exclude nephrolithiasis and to obtain a baseline for enhancement measurement.

Conclusion
Multidetector computed tomography imaging is the least invasive and most accurate method for preoperative evaluation,
aiding in surgical planning.
Illustrative cases from our department were gathered to demonstrate a step by step evaluation of the living renal donor and the main teaching points on each step.
