Aims and objectives
The incidence of the Renal cell carcinoma(RCC) is4 per 100000 people per year and its 5-year relative survival rates is almost 72 % between 2003 and 2009 in United States [1].Detection of the RCC is increased by frequent use of the cross-sectional imaging for other reasons,
and most of incidental RCC measured less than 4 cm (T1 stage) [2].To treat T1 stage RCC,
primarily partial nephrectomy (PN) was performed as standard treatment.
Recently,
considering general condition of the patients,
minimal invasive technique including radiofrequency ablation...
Methods and materials
This study was approved by the institutional review board.
Between November 2011 – November 2017,
pathologically confirmed RCCs were reviewed retrospectively.
We treated T1 stage RCC patients using LPN until January,
2015.
After that,T1 stage RCC was treated with fusion-imaging guided RFA.
Twenty cases were included in RFA group and twenty four cases were included in LPN group.
To evaluate its technical insufficiency,
we defined primary technical success as the percentage of patients who underwent complete response (CR) of tumor in CT within 48 hours...
Results
Both of primary technical success and technical effectiveness were 100% in RFA.
Disease-free survival rate (DFSR) was evaluated at 6 months,
1 year and 3 years.
In RFA group,
DFSR was 100% in each time.
However,
in LPN group,
DFSR was 100% at 6 months and 1 year while 93.8% (23/24) at 3 years.
Difference of the DFSR between RFA and LPN groups was statistically insignificant (P>0.05).
Mean value of the periprocedural creatinine level was decreased in RFA (-0.33mg/dL),
but increased in LPN (0.27mg/dL) (P<0.001)....
Conclusion
Fusion imaging-guided RFA provides equivalent disease-free survival rate with LPN in this study.
In addition,
creatinine level,
hospital days after procedure showed improvement result at the RFA with fewer complication than LPN.
Thus we offer that fusion imaging-guided RFA could replace LPN to treat T1 stage RCC patients effectively.
