Learning objectives
To be aware of smoking-related interstitial lung diseases as differential diagnosis of patients with smoking history, complaining dyspnea and/or non-productive cough.
To discuss clinical features and characteristic imaging findings of these diseases.
To emphasize importance of interdisciplinary cooperation between the departments of chest diseases, radiology and pathology for proper patient management.
Background
Smoking is one of leading causes of preventable early mortality/morbidity, worldwide. Cigarette smoke is complex mixture of more than 4000 components, of which about 200 are carcinogens, have been associated with numerous diseases involving entire body/systems. Since cigarette smoke is inhaled, it is inevitable that it will have significant effects on respiratory system. Inhalation of cigarette smoke causes many diseases associated with lung-respiratory tract/particularly chronic obstructive pulmonary disease/lung cancer. Among them, interstitial lung diseases are, also, important.

Findings and procedure details
1-RB-ILD:
Case-1 :45 y.o.a male/smoker with 80 pack-year/non-productive cough/exertional dyspnea for last 6 months. CT findings are consistent with RB-ILD (Figure-1).
2-DIP:
Case-2 : 43 y.o.a female-patient with 25 pack-year smoking history/complaining non-productive cough/finger clubbing. CT findings are consistent with DIP (Figure-2).
3-PLCH:
Case-3 : 58 y.o.a male/smoker with 50 pack-year history, with the complaints of clubbing/dyspnea. CT findings are consistent with PLCH (Figure-3).
4-IPF:
Case-4 : 75 y.o.a male/smoker with dyspnea. Lung auscultation reveals velcro crackles in bilateral lung bases. CT shows typical findings...
Conclusion
Smoking is most common preventable cause of early mortality/morbidity worldwide and has been associated with many diseases which include, also, interstitial lung diseases.
While there is strong epidemiological correlation between some of the interstitial lung diseases and smoking, scientific evidence is less accurate in rest, and cause-and-effect relationship has not been clearly established.
Although SR-ILD is rarer than other diseases caused by smoking, it may have of utmost importance. Among clinical, radiological differential diagnoses in smokers, keeping possibility of SR-ILD in mind is first step...
