Learning objectives
1.
To review the MR imaging of bursae at the shoulder with illustration
2.
To present MR imaging and illustration of various cystic lesions at the shoulder may encountered at daily practice,
with relation to rotator cuff,
glenoid labrum,
nerves,
bone and joint with or without pathology
Background
Cysts or fluid collection around the shoulder are very common.
Most are benign and related to the collection of fluid in the bursa,
synovium or ganglia.
MRI can help delineate the location of the cystic lesion in relation to anatomic structures.
Knowledge of the specific anatomic location of the lesion can help make a correct diagnosis.
Imaging findings OR Procedure Details
I.
Bursa around the shoulder (Fig1)
Awareness of location and extent of juxta-articular bursa is important in order to identify or exclude significant pathology.
1.
Subacromial-subdeltoid bursa
Location (Fig2)
..between the rotator cuff tendons and the coracoacromial arch
..between the rotator cuff tendons and the deltoid muscle
Function
..Covers a large surface area and facilitates movement
Clinical significancy (Fig 3)
1) Rotator cuff tear
2) Inflammatory or crystal deposition disease
3) Septic bursitis
2.
Subscapular bursa = superior subscapular recess
Location (Fig4):
..between the scapula...
Conclusion
1.
Know variable cause of cystic lesions around the shoulder
2.
Familiarize with typical imaging feature of cystic lesions on MR,
including fluid collection in normal bursae and pathologic cysts
