Learning objectives

Tobriefly review the ossification process ofthe ischiopubic synchondrosis. To review theimaging findings ofVan Neck-Odelberg disease. To summarily describe the most important differential diagnosis.

Background

Synchondroses are temporary joints composed of hyaline cartilage present during the process of skeletal maturation. With skeletal maturation, they become thinner and usually close by bony union or synostosis. The time of closure is variable, but typically begins in early childhood and is completed before puberty. Van Neck-Odelberg disease (also known asischiopubic osteochondrosis)describes radiographic abnormalities of the ischiopubic synchondrosis(IPS), a frequent finding in radiologic workup in children before puberty, during fusion of the pubic and ischial bones. Imaging findings OR Procedure Details

Conventional radiography TheIPSacts similar to a primary ossification center and its ossification is variable with respect to the rate and pattern of mineralization. Radiographic changes usually include a well-marginated expansile lesion, osteopenia, radiolucencies, and patchy calcifications, with a normal structure of the surrounding bone. It is a common finding at radiologic workup, with a reported prevalence by Ross and Caffey of about 50% in children 6 to 8 of age, and 20% in children from 2 to 19 years of age according to Palme(5). Conclusion

Van Neck-Odelberg diseaseshould be consideredin children before pubertywith groin or buttock pain. Besidesinflammatory markers and blood cultures,MRI findings can be helpful, depicting edema of the bone and adjacent soft tissue. Recognition of this entity may help prevent delay in the diagnosis, invasive procedures, and unnecessary treatment.
